£24,968 pa, 25 hours per week

We currently have a vacancy for a Carers Support Advisor to join our team in South West Surrey. In this varied role, you will split your time between working within the community and within Frimley Park Hospital.

Your work within the community (18 per week over 3 days)
Working in partnership with voluntary and statutory agencies you will provide information, advice, signposting and advocacy to adult unpaid carers about Care Act rights, learning & work, wellbeing and benefits.

Managing a caseload of carers, you will encourage carers to identify their own needs and facilitate initiatives for carers including running carers networks and support groups. This role involves visiting carers in their own home and travel across South West Surrey (Guildford and Waverley). You must have a current driving license and use of a road worthy vehicle.

Your work at Frimley Park Hospital (7 hours over 1 day)
Working in partnership with hospital colleagues on specific wards you will Identify and support carers (including Young Carers) in the hospital setting. You will also engage with hospital Health Care Professionals so that they are aware of our services to encourage them to refer Carers us.

You will act as an advocate where appropriate, attending multi–disciplinary meetings, hospital discharge meetings and writing letters and making phone calls on carers’ behalf and encouraging self-advocacy.

About You
You must have experience of working with and advocating for adults and families and have an understanding of carers rights. You will have knowledge of legislation relevant to carers and disabled people. You will be a confident communicator and be able to run training and information sessions to groups.

In the hospital setting, you will be working alongside NHS colleagues and ideally you will have an understanding of clinical ways of working, so that you can establish a presence in this busy, changing environment.

This role is subject to an adult enhanced DBS check.
